Description of Customization Types

Generic

Available in English or Spanish with Donate Life logo & URL/Done Vida & URL on the front and back with a blank inside. *No mention of DMV Appreciation Month to allow for year-round use!

Custom – Online Design (Free)

Available in English or Spanish. Using our design portal, you can make limited customizations to the front of the card: Replace the Donate Life/Done Vida logo with a Donate Life/Done Vida state logo, replace the URL with your state registry URL and/or add a second logo. Inside of card is blank.

Custom – DLA Design ($100)

You may choose to completely customize the inside of the card with a message, organization logos, names, etc., along with customization of the logos and/or URL on the front in which case Donate Life America will work with you directly and a $100 customization fee will apply. This custom fee also includes custom printed envelopes with your return address and/or logo (black and white only), if desired.* Please proceed to checkout for this option. Donate Life America will contact you once your order is placed to begin the customization process.
*You have the option of choosing blank envelopes in the "Custom Envelopes" field as a more economical option.

Custom – Reorder Existing Art

Select this option if your organization has already ordered a custom DMV Appreciation Note Card from DLA and would like to reprint it with no changes.
