Description of Customization Types

4 Panel | Generic

No Customizations. The National/generic version of the brochure has the Donate Life logo and mentions DonateLife.net on both the back panel and the fourth inside panel under “HOW CAN YOU HELP?”.

4 Panel | Custom – Online Design

Using our design portal, you can make limited customizations to the fourth inside panel (“HOW CAN YOU HELP?”) and the outside back panel which include: replacing “DonateLife.net” with your state registry URL, replacing the Donate Life logo with a Donate Life state version, adding a second logo, and/or replacing the Benjamin story box with your own picture and copy. If you would like to reprint a previously ordered “Online Design” job, please go to Your Jobs.

4 Panel | Custom – DLA Design ($100)

You may choose to do a completely custom back panel, in which case Donate Life America will work with you directly and a $100 customization fee will apply. Please proceed to checkout for this option. Donate Life America will contact you once your order is placed to begin the customization process.

5 Panel | Custom – DLA Design ($250)

As the most complex option, in addition to a completely custom back panel, you may choose to add two additional panels to include any of the following: a registration form, mailer panel, local images and stories, and/or more information about your state registry and organization. Donate Life America will work with you directly and a $250 full customization fee will apply. Please proceed to checkout for this option. Donate Life America will contact you once your order is placed to begin the customization process.

4 or 5 Panel | Custom – Reorder | Existing Art

Select this option if you have previously ordered a DLA Design version that you wish to reprint with no changes. If possible, please reference your previous job name in the “Notes” field.

4 or 5 Panel | Custom – Reorder | Updates to Existing Art ($40 / $60)

Select this option if you have previously ordered a DLA Design version that you wish to reprint, but need to have changes made to the art file prior to printing. Donate Life America will work with you directly and a customization fee will apply ($40 for 4-Panel Brochures, $60 for 5-Panel Brochures). If possible, please reference your previous job name in the “Notes” field.
